Default Offtopic questions - Nutrition/Stress Repertory

Some time ago during another discussion, I mentioned construction of a repertory of signs and symptoms of stress related nutritional deficits as part of my PhD dissertation. Many of you responded very favorably and encouraged me to publish. I'm finally at that point and would very much appreciate your input. I am considering using Lulu (self publishing) to keep the cost down and make the material available as a download as well as traditional paper version. [I've worked with major publishing houses on my previous books and have no burning desire to go through the months of working with a junior editor to get it into their format.] I am also currently considering putting the repertory 'up front' but still presenting the background research and documentation as part of the book - perhaps using small print to keep the page count down.

Your input on this process would be greatly appreciated.
a. Have you used Lulu or any other self/electronic publisher? Would you be willing to share your experience (what worked, what didn't, etc.)? Any guidance or information from layout to bookkeeping would be greatly appreciated.
b. I am considering 6x9 inch - but 8 1/2 x 11 inch is also possible - is there a preference among repertory users of a size that is most convenient? I will also be including blank forms I've developed for using this repertory - which is the only reason I'm currently considering the larger format.
c. As homeopaths - the word Repertory is immediately understood to us - but I find nutritionists are unfamiliar with the concept. Is there a better word that simply conveys the concept?
d. Based on your experiences as a homeopath - using repertories frequently - do you have any suggestions about formatting that would make the final product more 'user friendly'? My personal pet peve is that I always need room to make notes - and I want more 'blank space' to add things - but that will add to the page count (and cost).

While I do believe this is a topic of great interest to most practicing homeopaths - who among us doesn't see stress related symptoms??? - I recognize that this is off topic for this list, so I ask you to please ONLY respond off list -
