QUESTIONNAIRE FOR CHILDREN

Child's First Name:RUBY
Age:3
Height:98.5 CM
Weight:14.300 KILOS
Birth weight:3.900 KILOS
Hair color:BLONDE
Race/nationality:ENGLISH

1. What is the chief complaint? Include information about the side of the body that's affected; the color, odor and consistency of any discharges that may be present; whether the complaint came on gradually or suddenly; and all changes from the normal state, including mental/emotional changes, body temperature changes {colder or hotter than usual}, appetite and food and drink desires that may have changed; and try to be specific in describing the sensations involved in the complaint, for instance, "pain as if a splinter in the throat", etc.

THis is the 2nd time in the last few months she has had the following mouth condition. She had a fever for two days or so, but was quite chirpy and good-natured with it. Then she started getting a sore mouth again, her tongue went white {a little redder around the edges} with red spots on it and then she got a little pimple on each side of it. She also got ulcers inside her cheeks. it was too sore to eat and she became very irritable with hunger and pain. One night I finally gave her paracetemol {first time}. Around this time I started her on Borax 7ch every few hours but after a couple of days I didn't think there was any significant change. The glands on both sides of her throat swelled up at around this time. I half heartedly gave her Nit ac 7ch a few times after giving up on Borax, but I didn't really see it through as I was unsure .
Since then, she has been able to eat again finally and even got one full night's sleep so I feel things are on the up. However it is taking a long time, and now that her ulcers have subsided and her tongue looks quite a bit better, her gums have gone very red and spongy, they had boils on which seem to have gone now and behind and just between her two front teeth there is a a little nodule of gum like a a tiny balloon. Aware that perhaps I should be taking more care of her teeth { as this seems to be gingivitis?} i have been brushing them for her, but this makes her gums bleed. The glandular swelling has subsided. The other thing that seems to have subsided now is that she had an outbreak of spots on her nose. She still has a few and some reddish spots on her face {maybe from the heat} and a slight rash on the right side of her neck. Her breath still smells bad. She is asking for a lot of {cow's} milk at the moment. {We are also in the gentle process of weaning.} She also complained that her vagina hurt - at the beginning too, she was red there but tonight I had a look with a good light and removed a small amount of thickish yellowy discharge from there which smelled bad in the same way as her breath. Blood tests done but they are more or less normal {a little low on iron and sugar and blood pressure a little too high but doctor thought that was OK given she had been sick and not eating}.
Her diet went out the window after she couldn't eat and then she wouldn't have anything but chocolate icecream or cake {which I made as healthy as I could}. Then lots of yoghurt. Still lots of milk too. The only thing I have given her is a spray called Tantum which is a local disinfectant and pain reliever {doctor recommended}, I tried another thing which helps the gums heal, a synthetic enzyme thing {also what the doctor said} but she won't have it. And finally a propolis spray which she likes but I don't know if its the right thing...

2. When did this problem start? What happened in the child's life around that time? What do you think caused it?
1st time- nothing that i can think of -
2nd time- we had been on a trip to england, then back to spain, to a festival. diet a bit irregular and lots of moving and travelling and full time with me....

3. What aggravates the chief complaint {certain foods, weather, movement, light, noise, touch, pressure, music, heat, cold, open windows, closed windows, being at the sea shore, sympathy or anything else you can think of}?

Relatively good i think {though her father drinks a lot of alcohol}. Pregnancy-good, i felt healthy. Mentally very positive but upset by break-up of relationship with her father.

8. Did the mother have any unusual or memorable dreams while pregnant?
That i could swim with dolphins.

9. Did the mother take any drugs while pregnant?
A very small amount of cocaine one night before i discovered i was pregnant. Then the odd glass of wine.

10. Were there any complications at birth? Cervical disproportion - caesarian

11. At what age did the child: crawl, walk, talk, teethe, toilet train, wean? walk- 1 year. talk -1 and a half to 2 years. teethe- 8 months? toilet train - 2 years. wean - not yet.

12. How did the child react to the following situations: vaccinations, birth of younger sibling, tarting day care, starting school, spending night with a friend, going away to camp, traveling with the family?
day care - she was very confident and at ease. travelling - always been able to adjust quite well.

13. How many rounds of antibiotics has the child had and for what conditions? She may have had antibiotics via my breast milk when she was born as i had them intravenously during the birth. They may also have put some antibiotic in her eyes on her birth as regular procedure but I never found out.

14. Any skin conditions treated with cortisone-type cream? No.

15. Did the child have an especially severe childhood illness--measles, mumps, croup, etc.? She hasn't had any of these.

16. When ill or upset does the child want to cling or be left alone, or something else
altogether? Usually wants to be with me.

17. How would you describe the child's behavior when playing with other children? Very open and generally gentle and interested.

18. What feedback do you get from the child's teachers? That she is very independent. that she plays alone easily, and makes friends easily. that she is rather sensitive and gets upset if they ask her to do something even when there is no real sense of chastisement -like "can you put your shoes on to go outside?" for example and she sets upset as if they have told her off and can get very angry too. She loves looking after baby dolls. she is very gentle and sweet-natured most of the time but has a real angry temper on her too.

19. How does your child treat animals? with great respect and she is gentle.

20. What foods and drinks does your child crave? What foods is he or she averse to? Are there any
foods he or she reacts badly to and in what way? Do the drinks have to be cold {or with ice} or
can they be any temperature?
She likes yoghurt, chocolate, chocolate ice cream {not any other flavour though}, fish, meat, potatoes, cucmbers artichokes, leeks, broccoli, spinach, coconut milk, milk, egg white.
She dislikes carrots, peas, peppers, egg yolks,

21. What fears does your child have? None that i am aware of.

22. Is the child chilly or warm? Is there excessive perspiration anywhere? She gets hot easily.
She likes cool water and being in the shade.

23. How affectionate is the child when not sick? She is pretty affectionate.

24. How sympathetic is the child {concerned with the suffering of others}? Very much with her baby dolls and if anyone cries, she is really concerned.

25. How is the child affected by music and dancing? Loves music and dancing

26. What emotion tends to predominate with the child? Humour.{is that an emotion? if not, curiosity and generosity.<}

27. Is the child fastidious? Please explain. No though sometimes she will pick out foods she likes and leave those she doesn't.

28. Is the child sensitive to criticism and reprimand? Give examples. Extremely. She will sometimes understand a neutral request as a criticism as i mentioned above.

29. Describe the child's eating habits, for example: picks at his food, or eats voraciously, or
is full after 2 bites, or can't sit still to eat, or must be fed or he won't calm down, and so
on.
It has varied a lot over the last year, its difficult to say. She used to sit down and eat quite well now i find she dioesn't really unless we pretend she's a baby who needs to be fed. Sometimes she picks, occasionally she is voracious. She finds it hard to sit still especially as there is no big family eating occasion, she tends to be on her own with me sitting with her.

30. Are there any digestive complaints--waking with stomach pains, or a lot of gas and bloating or burping, or constipation, etc.?
No

31. Are there any problems associated with sleep? What position does he sleep in? Does he throw
the covers off? Does he sweat during sleep? On which part of the body? Trouble falling asleep?
Nightmares, etc.?
She has never really slept through the night but constantly wakes up for breast milk or milk or
because she wants to sleep with me. She often sleeps on her front with her bottom up in the
air.She grinds her teeth very loudly and it sounds painful { this i noticed prior to her becoming ill for the 2 weeks we shared a bed when we were travelling around.} She always throws offf the covers. She dislikes the idea of going to bed. Her head is sometimes sweaty, but then it is hot here.

32. Any skin symptoms or anything note-worthy about the skin, the complexion, etc.? Usually very clear with that breastmilky glow. But now, very wan and pale and unhealthy looking.

33. Is there anything striking, or characterizing about your child, for example: attatchments,
unusual desires and abilities, preferences in clothing, etc.? She is learning 3 languages at a time and I think she is verbally very precocious in her mother tongue {English} even if that were not the case.
I don't think she is particularly attached to any of her teeddy bears etc, maybe for a few days then a different one but there has never been a special tedy or thing she wanted to always have with her. {She likes her realistic baby best though}.
She likes flamboyant princessy clothes but then I guess most 3 yr old girls do...

34. How cooperative is the child? Very, when she wants to be. She is incredibly wilful if she doesn't want to co operate.

35. What's standing in his way of getting what he wants or needs? I hope she is getting what she wants and needs, but danger or my needs will sometimes get in her way! Maybe at nursery she is disadvantaged by not speaking the language so well and may find it frustrating to communicate when her level of English is so developed. It must be harder for her then to get her needs met.

36. How often does the child get sick? Does he tend toward certain illnesses?
She has had the odd cold, but basically her health has been wonderful except for this mouth thing last june that has now recurred. And actually i was worried for quite a few motnths as she seemed to have a chronic cough at night which i even thought might be asthma but it seems to have gone in the last few months. I wonder if that is related to her present condition now that I remember it…

37. What illnesses run in the family, including the grandparents?
Cancer, grandparents on both sides.

38. What does the child really love to do?
Play on the beach. run around near and in water with no clothes on. Hide under sheets/blankets. Look at animals. Be with babies. Go to parties.

39. It's important for us to know what may have been the cause for the complaints you have mentioned. To this end, please make a list, in order of occurence, of the major events in the child's life, including such things as physical and emotional traumas, major illnesses, losses, deaths in the family, surgeries, etc. and the child's age at the time. If he or she had a notable reaction to these events, please describe.
5 weeks - she started crying for a few weeks solidly. Later i found out she had had vaginal thrush {probably because of the antibiotics i had intravenously during her birth}
3 months - she fell off a bed. Cried a lot, then was fine. No damage.
4 months - we went travelling in the Philippines and Thailand for 6 months. She seemed very happy and well adjusted, though we both got sick directly after the two long aeroplane flights there and back. At this time she had a severe cold and cough.
No traumas, major illnesses, losses {she did have an au pair for a year {from 1-2years} and now she has another one but we have been to visit her first friend and she seemed to adjust well to her leaving}, no deaths or surgeries so far...

THANK YOU!!!!!

What do the rest of you think?

Dear Zanna,

I do think that carc. is her const. remedy. But, I think there might be an issue with the body's PH, whether it's from the early antibiotic use or what, but, you can try the cell salt Nat-phos., which is supposed to regulate the acid/alkaline balance. See if that helps any. Also, you said you used nit-ac. for a short while and got some positive results from it. This is our main remedy for ailments from antibiotics. If Nat-phos doesn't help, you could continue with Nit-ac for a while and see how that goes.

Too many of these questions are leading - they actually mould the answers along the lines of Sympathetic, Anxiety for Others, Senstive to Reprimands, Fastidious, Love music/dancing - you will get lots of Carcinosin using questions like that.

Doing some repertory work brings these results:

Sleep position face and knees
Grinding teeth in sleep
Kicks off covers
Paleness of skin
Sensitive to reprimand/reproach
Worse heat
Tantrums
Craves sweets
Pain in mouth on eating (although this is common for ulcers of course)

the only remedy that has all these symptoms is Cina.

The anger is there - I think it is the questions that were leading away from it.

"....and she sets upset as if they have told her off and can get very angry too. She loves looking after baby dolls. she is very gentle and sweet-natured most of the time but has a real angry temper on her too."

Since the questionare is not probing for temper problems, and this is the eternal problem with a static questionare anyway (no ability to pick up on and explore a stated symptom),it could be missed.

And Carcinosin children can look like Chamomilla too! They may have very bad tempers.

Hi David,

Cina's a good idea and something for Zanna to keep in mind, thanks! Maybe I have a poor understanding of cina, because I always thought that cina children were like a more severe chamomilla in terms of the irritability and so on, and this child seems rather even tempered, but thanks for looking at the case and giving your advice.

Do you have much experience with carc. children? The two I've seen have both been "Tom boys"--an expression used here meaning girls with masculine traits, such as excelling at boy's sports, etc. What's the cause of the temper you mentioned?
