I kept coming back looking for an answer to this question about the girl. Imagine my great surprise at finding you having corrected me and made it so simple. I am flattered! I have a whole heap of questions, though. I’ll present them separately.

Thought I'd penetrated the case as far as could be done as presented. Because it sat fallow for so long and I'd invested so much study into it, I posted it expecting a yes or no back from her about open fontanelle and to then use what parts of the case analysis remained or changed. I’d seen the two rubrics you used but considered spongy another manifestation of open fontanelles and the other one meaningless here since an open fontanelle would be painfully sensitive. I must have forgotten to incorporate the former rubric into the list of rubrics for open fontanelle as Symptom #1. But I don't think I understand some things about your analysis.

I see you’re saying I assumed it was spongy inward due to missing bone tissue rather than swelling outward due to what? I mildly considered swelling, but of what tissues? Of the two possibilities, both of which are very strange, I could conceive of no mechanism for swelling of bone or skin tissues that could be called “spongy.” I furthermore thought I’d somewhere seen a subrubric named “reopened” with one or two entries, and that told me it can happen, whatever the mechanism. Besides, bone loss is a constant problem with aging later in life, so a mechanism does exist there. Why and how localized are other matters, but their resolution could not help us find the remedy, only possibly prevent it later in others. That's either very dumb of me or insightful of you, and I already know the former and latter but not the answer here. Swelling of what tissues? what tissues can swell at the vertex?

Hello Hans,

I just thought of a mechanism for swelling at the vertex: leakage of cerebrospinal fluid.

I've seen that in concussions at the cranial sutures from accidents and injuries.

Swelling is an understatement for it due to the hydrostatic nature of the stuff.

Another question to the girl therefore should be: has she suffered an injury there?

Also, I am not sure she would have had to have suffered an injury there, for my arguments of evidence of bone loss (osteo-loss, what is it?) shows it is another mechanism.

Are you arguing that the swelling is from leakage of cerebrospinal fluid?

Chilliness, however, remains another question if it is.

Can you shed light on either of these matters.?
