What I feel in your case here that your patient might be suffering from Acute Glomerulonephritis. For instance look at the following homeopathic remedies

Apis is the top most remedy in Acute Glomerulonephritis. then come Vesicaria.

After acute Glomerulonephritis, the next stage is Renal Failure. For this purpose Ars Alb have good clinical responses.

If the patient is feeling severe pain with Acute Glomerulonephritis then don’t forget belladonna and mag phos.

If protein symptom is more prominent in Acute Glomerulonephritis then Serum Anagalis will be more helpful then any other remedy.

Acute Glomerulonephritis with yellow color urine. Strong smell then Benz Acid and Borax.

Nit Acid, when scaring process will start in Acute Glomerulonephritis.

If Hematurea – microscopic or gross symptom is more persistant in Acute Glomerulonephritis then Thala Burs, Sars, Mille and Nit Acid will come against you.
