We must always be on the alert for possible harmful influences, but the mere fact that a billion people worldwide use cellphones regularly and all surveys are at worst inconclusive tells us that even if there is a harmful effect, it must be rather small. Otherwise, it should have shown up by now. However, as noted, long-term effects are still possible.

Those cannot quite be compared. First off, for many decades, we did not have the statistical tools to discover the ill effects of smoking. Secondly, that danger has been realized for half a century, it is just that effective lobbying from the tobacco industry, and general denial from an addicted populace, meant that the knowledge was largely ignored.

Except for some quite old models, the radiation from different models is in the same general magnitude. For possible long-term health effects, whether it emits 1/4W or 1W is of minor importance.
