Speaking as someone who endured a series of ear infections as a child, I suggest that you want to use every possible means of treatment - including antibiotics.

An ear infection can be serious, and if it isn't cured could lead to permanent loss of hearing. Also, if you suspect that the child has ruptured an ear drum then I would advise seeing a doctor as if the ear drum has been ruptured then there is an increased chance of an infection in the inner ear. (I speak from experience having ruptured an ear drum more than once).

And lastly, I think that the internet isn't a good place to find urgent medical advice. Its much better if your child were to be looked at in person.
