"dwarfish can have many meanings."
It would be much better if it was clearified what 'dwarfish' means to the patient. It could mean shorter than others, or small or freakish, feeling of odd or different because of being a woman in a man's world. Perhaps she has overextended herself, studied too hard and isn't accepted like or on the same level as the men are. Is this what makes her feel dwarfish? She could be asked to put it in different terms or give an illustration of what she means or more expressions of how she feels.
