Aphorism 250 basically states that a medicine which is poorly selected can be antidoted by one which is homeopathic to the case (that is, selected more carefully, taking into account the old symptoms as well as any new symptoms brought up by the "wrong" remedy).
What we have come to document as clinical infomration based on this process--what you now can see as "antidotes" for specific remedies--is based on gathered information from this process of remedy administration and case management.
