Before all these holistic doctors came on board, I had to go the religious exemption route.

I did exactly what I was told by one of the anti-vaccine societies:

I typed out a statement which said something like, "I object to vaccinations on religious grounds as per section ___, subparagraph ____ of the Pennsylvania law stating: blah blah blah."
I then had it notarized--several copies--and the school accepted it.

There are anti-vaccination societies that will tell you exactly what to say or write for your particular state. I suspect they can be found online.
