I was wondering if you use this questionnaire? Some of the questions seem better asked face to face in order to gain a better picture through the patient's reaction. Like a "no, not much" on paper might be a "no" with a wrinkled nose and a whole body shudder in person. Just curious about that, though I did print it out and am definitely filing it for future use!
