I agree with bwv11 that the parents are almost certain to be a factor of some sort, even if I don't believe in the exact copying of someone you dislike, rather that the child tries to attract (negative if any) attention or act as a vent for unspoken problems in the family. So try to convince the parents to have treatment as well. It doesn't necessarily have to be with you, they can seek another homoeopath or some other form of therapy if they want, but you could use as an argument that they are so exhausted already that they need help for themselves, or that by having treatment they become better parents (no doubt they feel guilty at some level). You can use the analogy from flying emergencies, that if you have a child with you, you first put on the oxygen mask, before you put it on the child - as there's only temporary help in the child having some oxygen if you're already dead of oxygen deprivation and can't help the child survive if the plane should come safely to the ground or land on the ocean.

Try also to find info from Paul Herscu or his wife Amy Rothenberg (www.nesh.com), they are both really good with "difficult" children. Paul often lectures in Norway, so it could be really rewarding for you to try to get to one of his seminars on children, as it's not so far for you to go.
